    Service form customization

    A) How can I use customized service forms as our default ticket form.
    B) I would like to print on the ticket the date and time when the appointment is scheduled. Is this simply inserting a field?

    You can set which service form will be printed as the default from Tools > Options > Tickets > Default Detailed Report.

    As for adding the Appointment time to the Ticket service form, you can read on how to do this in this Forum discussion.
